Supreme Gourd is obtained by completing [Yuan Shoucheng](/yuan-shoucheng) questline and having a [Medicine Buddha Gourd](/medicines/medicine-buddha-gourd). You will need to speak with Yuan Shoucheng in each of the first five chapters. You also should upgrade [Old Gourd](/old-gourd) to [Medicine Buddha Gourd](/medicines/medicine-buddha-gourd), a +9 gourd. After speaking with Yuan Shoucheng in Chapter 5, you can interact with the fire in the middle of the room where he is, and that will turn the Medicine Buddha Gourd into a Supreme Gourd.
