To obtain the Five Skandhas Pill, you will need to do Mandrill Chiefs questline, which requires you to obtain the five skandhas.
First you will need to collect the four Skandhas: Skandha of Form in Chapter 1, Skandha of Feeling in Chapter 2, Skandha of Thought in Chapter 3, and Skandha of Choice in Chapter 4. Once you have all four, you can find Giant Shigandang in Chapter 6, who will turn hostile (without the four Skandhas, he won't be hostile). Once the boss is defeated, climb on him to get Skandha of Consciousness. Now will all five Skandhas go talk with Xu Dog and choose "Show the Five Skandhas", then "Make Five Skandhas Pill". Xu Dog will then make the pill and the Destined One will automatically consume it.
