Acquired by crafting your first Medicine at Keeper's Shrine or Xu Dog. To be able to craft medicine, you will need to obtain formulas. The first formula for Evil Repelling Medicament is obtained by speaking with Xu Dog in Yellow Wind Ridge, then defeating the frog next to him and talking with him again. After that you can access the "Make Medicine" menu at the Keeper's Shrine to craft an Evil Repelling Medicament.
